New Anthem health insurance cards coming

Anthem, the University's new health plan administrator, has sent new health insurance cards to the homes of eligible faculty and staff. The cards are enclosed in blue and white envelopes like this one.

Follow these steps to access a digital version of your new health insurance card or request a new one if the card is not received by the New Year or if it is misplaced.

  1. Contact Anthem’s University of Notre Dame direct customer service line at 833-835-2717 to obtain your Anthem member ID. They will ask for your Social Security number, first and last name and date of birth to give you your Anthem member ID. The ID provided will be in the following format: ND + the last 7 digits of your NDID. Example: ND8765432.
  2. Create an account on http://anthem.com using the Anthem member ID provided to you during your phone call with Anthem.
  3. Once you have created an account, scroll down and click on “Request ID Card” and/or “Print Temporary ID Card” under the Resources section.

Once you register online, you can download and access Anthem’s mobile application (Engage) on your smartphone. Engage will allow you to display your insurance card directly on your smartphone. You must register on a computer before you can log in to the Engage application. Below you can see what the application icon will look like if you search for it with your smartphone.

**Note about Notre Dame Group Number:
The Notre Dame Group Number you receive will depend on the plan you elected (PPO, HMO or HDHP). This information will be provided on your insurance card. Should a provider need it for treatment early in the New Year before your card has arrived, you can use the following group number based on your elected plan:

PPO: IN2021M001
HMO: IN2021M005
HDHP: IN2021M003
